Job Description:

Provide approximately 45-minute in-home health evaluations
Engage one-on-one with health plan members and answer health questions
Review medications and medical history
Conduct physical evaluations
Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry
Build trusted relationships and help improve health outcomes
Visit people nationwide and connect them to appropriate care
Fulfill the requirements of the specified clinician role with Signify Health
Travel locally and within licensed states as needed
Conduct virtual visits as needed
Provide Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screening

Requirements:

Active, unrestricted license(s) in coverage area(s)
Board certification (required)
Multi-state licenses (preferred)
Nurse Practitioner or Physician Assistant qualification
Ability to travel locally and within licensed states, as needed
Ability to conduct visits in private residences that may contain household pets, secondhand smoke, and airborne allergens or irritants
Ability to use an iPad and intuitive clinical workflow
Anticipated weekly hours: 12
